Registration & Contest Forms

These PDF forms are only available to use as hard copy printouts. You must have the free Adobe Acrobat Reader to view and print this file. Please mail them to the addresses listed on the individual forms.

• SSC 2008 SEMINAR REGISTRATION FORM (516k)
  Available for printout. Send this form with your registration fee (or a copy of your PayPal Confirmation page) separately from the contest entry form (if you enter the photo contest) below.

  Available for printout. Send this form with your contest fee (or a copy of your PayPal Confirmation page) separately from the Seminar registration form (above).
  
  

Online Credit Card Payment via PayPal

If you would prefer to pay for the Seminar Registration and/or the Photo Contest Fee with a credit card, you may do so with our online payment form. You will need to have a PayPal account to do this (which is free and easy). If you are not already a PayPal member, you will be prompted to sign up during the process.

Note that you will still have to mail the above forms in to their respective addresses, along with a printed copy of your PayPal Payment Confirmation page so your Contest entry and/or Seminar Registration can be processed. This is just a payment option for those who would rather use a credit card than a check.
